Fees

If your insurance is an HMO or requires a referral, the referral must be presented when you arrive to your appointment.  If you do not have your referral, you will be asked to reschedule.  If you are receiving cosmetic services or do not have medical insurance, payment is required at the time of service.  Please make sure you have proof of insurance and photo ID at every visit with our office.

No Show Fees

If you cannot keep your appointment, please notify us at least 24 hours before your appointment as a courtesy to our other patients who may be waiting for an appointment.

A service charge of $50.00 will be billed to patients who do not notify us within twenty four hours in advance to cancel their appointment.

Referrals

HMO’s, POS and CHOICE option plans require or suggest that patients use REFERRALS to reduce the patient’s out of pocket cost.

Under the terms of your insurance contract, it is the patient’s responsibility to bring referrals and/or authorization numbers at the time of your visit, or have it faxed to our office prior to the visit. If the referral or authorization numbers are not available at the time of your visit you will be asked to reschedule your appointment. This policy will help reduce undue cost to you.

If you wish to be seen without a referral, payment in full at the time of service will be required and a signed waiver of your insurance rights will be required.

We will do our best to help you understand this process. However, since we do not have control of how your referrals are loaded or used by your insurance carrier we cannot be responsible for services denied. Some primary care physicians require up to 48 hours or longer to process a referral request. You need to check with your insurance carrier or primary care physician before each visit to ensure that you have a valid referral on file.

Co-Payments and Balances

We participate in a number of managed care and PPO plans. We will file your insurance if we have the correct information and the claims will be processed according to your benefit guidelines. All co-payments are due at the time of your visit. Any deductibles or co-insurance balance on your account will be collected at your visit. You are responsible for payment of your medical care within a reasonable time, regardless of status of claim. Services not covered by your insurance are your responsibility.

If your insurance requires a co-payment (usually indicated on the identification card), this payment must be made at time of service. Balances billed and remaining on account greater than 45 days may be billed a 20% collection fee.

All copayments and balances on your account will be collected at your visit. Please come prepared or your appointment may be rescheduled.
